Visual Quality In Patients With Different Dark Pupil Diameters After Visian Implantable Collamer Lens (Icl V4c) Implantation:An Observational Study

Purpose

This study aimed to compare the visual quality after implantation of implantable collamer lens (ICL V4c) with different dark pupil diameters.

Setting

Dongguan Aier Eye Hospital

Methods

90 eyes of 90 patients who underwent ICL V4c implantation in refractive surgery center in Dongguan Aier Eye Hospital from October 1, 2022 to June 30, 2023 were selected and divided into three groups. Group A was the group with a dark pupil diameter less than 6.5 mm, Group B was the group with a dark pupil diameter ranging from 6.5 mm to 7.3 mm, and Group C was the group with a dark pupil diameter more than 7.3 mm. Each group contained data from 30 eyes, and the right eye was selected for statistics. Patients' pre and post-ops UCVA and SE were observed and recorded for 3 months. Objective visual quality parameters were measured using an OPD-Scan III aberration analyser with pupil diameters of 4 mm and 7 mm.

Results

The 3-month postoperative UCVA and SE for all three groups exhibited a marked enhancement in comparison to their preoperative values (P<0.001). At a pupil diameter of 4mm, no substantial discrepancies were discernible in the HOAs among the three groups prior to and three months subsequent to the surgical procedure (P>0.05). However, at a pupil diameter of 7mm, group A demonstrated statistically significant alterations in HOAs and Trefoil relative to both preoperative data and data collated three months post-surgery for all three groups (P< 0.05). Both group B and group C exhibited significant variations in HOAs, Coma, Trefoil and Spherical aberration relative to preoperative assessments (P<0.001). 

Conclusions

Visual quality improved in both light and dark environments early after ICL V4c implantation. In the dark environment, HOAs, Coma, Spherical, SR and AR exhibited significant correlation with the preoperative dark pupil diameter, whereas Trefoil has no correlation with both the preoperative dark pupil diameter.
