10-Year Clinical Outcomes Of V4c Implantable Collamer Lens Implantation: Longitudinal Analysis Of Visual Acuity, Endothelial Cell Density, And Vault Dynamics

Purpose
This study aimed to evaluate the long-term clinical outcomes of V4c Implantable Collamer Lens (ICL) implantation for myopia correction over a 10-year period.
Setting
Retrospective observational study
Methods
The retrospective observational study, based on clinical practice, encompassed 103 myopic eyes from 54 patients. We assessed visual outcomes, including visual acuity and manifest refraction. Annual measurements of endothelial cell density (ECD), vault, anterior chamber depth, and intraocular pressure (IOP) were examined using mixed-effects models.
Results
At the 10-year mark, mean logMAR UDVA and CDVA were -0.037 ± 0.052 and -0.041 ± 0.046, respectively. Efficacy and safety indices stood at 1.11 ± 0.13 and 1.12 ± 0.12. CDVA loss of one line was noted in 2% of cases; however, 59% saw gains. MRSE was within ±0.5 D and ±1.0 D for 74% and 99% of patients, averaging at -0.19 ± 0.41 D. ECD and vault exhibited a slowing exponential decay over time, with rates of 0.012/year and 0.056/year, respectively. Lower ECD outliers had notably lower baseline ECD. No cataract, angle closure, glaucoma, or ICL removals reported.
Conclusions
Ten-year outcomes of V4c ICL implantation indicate effective and safe visual results. Both ECD and vault exhibited an exponential decay pattern over the decade. Patients with lower preoperative ECD particularly require careful monitoring post-implantation.
