Patient-Reported Outcomes And Optical Quality Assessment Following Bilateral Implantation Of Enhanced Monofocal Intraocular Lenses: A Prospective Case-Control Study

Purpose

To evaluate and compare patient satisfaction and optical quality through aberrometry after bilateral implantation of an enhanced monofocal intraocular lens (IOL) – Tecnis Eyhance™ (model ICB00) – and a conventional aspheric monofocal IOL - Acrysoft®IQ (model AU00T0).

Ophthalmology Department – Unidade Local de Saúde São José

Methods

Prospective case-control study including patients submitted to bilateral sequential phacoemulsification with implantation of an enhanced monofocal IOL (Group 1) or conventional aspheric monofocal IOL (Group 2). One month after the second eye surgery, binocular corrected distance visual acuity (CDVA), uncorrected distance visual acuity (UDVA), uncorrected near visual acuity, and distance-corrected near visual acuity (CNVA) were assessed. To evaluate the patient quality of life and self-perceived visual function, a questionnaire (Catquest-9SF) was applied. To determine optical quality, total high-order aberrations (HOA), spherical aberration (SA), coma and trefoil were assessed by wavefront topography ocular aberrometry (Visionix VX110®).

Results

Forty-eight eyes of 24 patients were included. The average postoperative UDVA (0.09±0.06 vs 0.11±0.05, p=0.634) and Catquest-9SF results (p≥0.05) were not significantly different between groups. Wavefront aberrometry assessment revealed that total HOA, cornea HOA and internal HOA were similar between groups, in photopic and mesopic conditions (p>0.05 for each). Coma and trefoil were similar between groups. Despite similar corneal SA between groups (p=0.371, p=0.167), total SA (photopic -0.03±0.10 vs 0.05±0.08, p=0.034; mesopic -0.21±0.08 vs 0.17±0.11, p=0.044) and internal SA (photopic -0.07±0.03 vs -0.01±0.03, p=0.019; mesopic -0.38±0.10 vs -0.09±0.03, p=0.038) were significantly more negative in the Tecnis Eyhance™ group.

Conclusions

The enhanced monofocal IOL evaluated provided great distance visual outcomes, comparable to a standard aspheric monofocal IOL, without a negative or positive impact on the self-perceived quality of vision. No significant increase in ocular high-order aberrations was found, except for the expected SA.
