Reasons For Not Performing Corneal Refractive Surgery: Outcomes Of A Tertiary Eye Clinic

Purpose

To identify the characteristics and reasons for elimination of the patients who attend a tertiary referral clinic for corneal refractive surgery (CRS).

Setting

Basaksehir Cam and Sakura City Hospital, Istanbul, Turkey.

Methods

A retrospective review was performed on the records of patients who were admitted to the refractive surgery clinic for CRS between January 2021 and January 2024. Demographics, examination findings, and reasons for elimination were recorded.

Results

The study included 905 patients, of whom 693 (76.5%) underwent CRS. Surgical indication was given to 519 (57.3%) of the cases at first examination. In the remaining 174 (19.2%) cases, indication was given after treatment. Among them, 162 (17.9%) had punctate epithelial erosions and blepharitis, 10 (1.1%) had corneal warpage, and 33 (3.6%) had irregularities in corneal topography. Of the 212 patients who did not undergo CRS, 21 (2.3%) preferred not to proceed with surgery, 33 (3.6%) had keratoconus, 43 (4.7%) had suspicious topography, 2 (0.2%) had a sibling with keratoconus, 76 (8.4%) had high refractive disorders that exceeded the corneal thickness safety limit, 17 (1.9%) had refractory dry eye and 20 (2.2%) had presbyopic complaints.

Conclusions

Ocular surface problems are prevalent among candidates for CRS. After receiving appropriate medical treatment, most of these patients can be treated with CRS. The main reasons for elimination from CRS are suspected ectasia and refractive errors that exceed the corneal thickness limit.
