An Experience Of Previously Centrifuged Triamcinolone Acetonide Suprachoroidal Injection In The Treatment Of Inflammatory Cystic Macular Edema

Purpose

To perform clinical and functional analysis of the results of an injection of 4 mg previously centrifuged, preservative-cleaned triamcinolone acetonide in the suprachoroidal space for the treatment of inflammatory cystic macular edema.  

Setting

In 2023, we have treated 66 eyes with cystic macular edema at the background of recurrent uveitis and postoperative cystic macular edema caused by chronic cyclitis after intraocular surgery or cyclodestructive operations. There were 20 males, 46 females, mean age 48.6+19.3 years; 88.7% of eyes had recurrent non-infectious uveitis, and in 11.3% of cases there was previous intraocular and/or cyclodestructive surgery. Follow-up period was from 3 to 9 months.

Methods

Under topical anesthesia in the operation room an injection of triamcinolone acetonide 4 mg into the suprachoroidal space was performed with a 30G needle. In 21 case injection was performed in combination with various intraocular interventions.
 

Results

Mean BCVA has increased from 0.33+0.23 to 0.46+0.27. Intraocular pressure was normal in all patients. Central retinal thickness has decreased from 478+152.8 мкм. до 316+119 microns, macular volume has decreased from 8.8+0.92 to 7.8+1.19 mm3. The results were influenced by presence of epiretinal membrane, DRIL, character of cysts in the neuroepithelium, condition of ellipsoid zone and retinal pigment epithelium. In cases with triamcinolone acetonide injection within other intervention there were no inflammation recurrencies and macular edema increase during 3 months.

Conclusions

Suprachoroidal injection of previously centrifuged, preservative-cleaned triamcinolone acetonide in a dose of 4 mg is a safe and effective method of inflammatory macular edema treatment. It is also a well-established medication support of complex reconstructive surgery in eyes with existing macular edema or high risk of its postoperative development.
