Clinical Outcome After Duet Procedure

Purpose
To evaluate the postoperative visual outcomes, safety, efficacy and patient satisfaction in DUET Surgery. Patients were primary implanted with either the RayOne Aspheric monofocal or RayOne Toric in the capsular bag, and the Sulcoflex Trifocal supplementary intraocular lens in the sulcus (Rayner, UK)

Methods
This retrospective, single-center, nonrandomized case series included patients who underwent cataract or clear lens exchange surgery with implantation of Sulcoflex Trifocal supplementary IOL (Model 703F) in the ciliary sulcus in supplement of Rayone monofocal or toric in capsular bag to achieve spectacle independence. Postoperative outcome measurements included refraction, monocular and binocular uncorrected and distance-corrected visual acuity for near and distance, as well as patient satisfaction. One-to-six month data are presented in this study
Results
Data from 145 eyes were analysed. The mean spherical equivalent was 1.43 D ± 2.53 D (range -8.75
to +8.50 D) preoperatively and reduced to mean -0.07 ± 0.40 D (range -1.25 to +1.25 D). Postoperatively, mean monocular uncorrected distance visual acuity (UDVA) was 0.10 ± 0.10 logMAR.
90.1% of the patients acheived monocular near visual acuity of P2 or greater (equivalent to 0.18 logMAR or better), enabling them to be free from the dependence on spectacles for reading tasks.
32 patients underwent early capsulotomy with the YAG Laser. There was high spectacle independence and patient satisfaction with visual outcomes. There were no significant adverse events reported.
Conclusions
Today, the proportion of multifocal implantations is not increasing, despite ongoing optical innovations aimed at improving the performance of multifocal implants and mitigate associated side effects. This underlines the difficulty of predicting and addressing patient dissatisfaction after multifocal pseudophakic implantation. The DUET procedure provides a more ethical alternative, offering a reversible solution with lower risk of complications in the event of intolerance or complications that may necessistate an explantation. .
