Purpose

To evaluate patients’ spectacle indipendence and visual outcomes after bilateral implantation of an advanced monofocal IOL compared to bilateral implantation of a monofocal IOL

Setting

Ophthalmology Clinic, Department of Neurosciences, Biomedicine and Movement Sciences, University of Verona, Verona, Italy

Methods

This randomized single blind clinical trial included 30 patients without ocular comorbidities other than cataract who underwent cataract surgery. Fifteen patients received bilateral advanced monofocal IOLs (LuxSmart) while the other 15 patients were implanted bilaterally with a monofocal IOL (LuxGood). Three month follow-up included: distance corrected and uncorrected visual acuity at 4m (DCVA and UDVA), 80cm (DCI80VA and UI80VA), and 40cm (DCNVA and UNVA), binocular defocus curves, contrast sensitivity (CS), subjective and objective (Root mean square-RMS, point-spread-function -PSF- Strehl ratio) visual quality, and the NEI RQL 42 questionnaire was administred.

Results

In the advanced monofocal group binocular UDVA, UI80VA and UNVA were -0.13±0.2, 0±0.1, and 0.26±0.15 logMAR, respectively. The values obtained in the monofocal group were -0.05±0.06, 0.05±0.1, and 0.37±0.15 logMAR. The DCVA showed higher performances in both groups. All mean visual acuities at defocus curves were lower than 0.1 logMAR between +0.50 D and -2.00 D for the advanced monofocal group, while it was lower than 0.0 logMAR between +0.50 D and -1,50 D in the monofocal group. In both groups CS were higher binocularly. Subjectively, the patients were more satisfied of their far and intermediate vision and were more spectacle independent in the advanced monofocal group.

Conclusions

In this study both groups achieved very good visual acuities at all distances, with the advanced monofocal group performing better in the intermediate distances. The advanced monofocal group experienced the highest grade of satisfaction after three months and resulted to be more spectacle independent.
