Refractive Outcomes, Visual Acuity, Rotational Stability, Decentration, And Tilt Of A New Toric Enhanced Monofocal Hydrophilic Acrylic Intraocular Lens: A Retrospective Analysis

Purpose
To assess refractive outcomes, visual acuity, rotational stability, decentration and tilt of a new toric enhanced monofocal hydrophilic acrylic intraocular lens (IOL).
Setting
This single-center, retrospective study was conducted at the Department of Ophthalmology and Optometry at the Medical University of Vienna.
Methods
Patients with preoperative corneal astigmatism exhibiting 0.75 diopters (D), as determined by AS-OCT (MS39, CSO, Firenze, Italy), who underwent cataract surgery and received mono- or bilateral implantation of a RayOne EMV toric IOL (Rayner, Worthing, UK) were included. At 6 months (6m), subjective manifest refraction, uncorrected distance visual acuity (UCDVA), uncorrected intermediate visual acuity (UCIVA) at 66cm, and uncorrected near visual acuity (UCNVA) Jaeger were assessed. Rotational stability, lens decentration and tilt were evaluated using AS-OCT Casia 2 (Tomey, Nagoya, Japan) at 1 hour (1h), 1 week (1w), 1 month (1m) and 6m postoperatively. Data is expressed as mean± SD [min;max].
Results
In this study, 31 eyes of 20 patients were analyzed. Mean preoperative corneal astigmatism was +1.85±0.59 [1.0;4.0] D, and postoperative refractive astigmatism at 6m was +0.42±0.33 D [0.0;1.25] with 95% of cases ≤1.0 D and 80% ≤0.5 D. At 6m, logMAR values for UCDVA and UCIVA were 0.12±0.10 and 0.20±0.12, and mean UCNVA was 2.3±1.3 Jaeger. Mean absolute rotation from intended axis to 6m was 3.9±3.3°. Rotations from OP to 1h, 1h to 1w, 1w to 1m, and 1m to 6m were 2.9±2.5°, 1.3±1.2°, 1.8±2.1°, and 1.6±1.5°, respectively. Mean decentration and tilt preoperatively were 0.17±0.85 mm and 5.38±1.82°, and after 1w, 1m, and 6m, they were 0.22±0.12 mm and 5.44±1.31°, 0.23±1.38 mm and 5.66±1.89°, and 0.24±0.15 mm and 5.99±2.04°, respectively.
Conclusions
Refractive outcomes were highly satisfactory, with a mean absolute residual refractive astigmatism below 0.5 D. Patients demonstrated good uncorrected visual acuity across distance, intermediate, and near vision. The RayOne EMV toric IOL exhibited good rotational stability and demonstrated low postoperative decentration and tilt.
