Purpose
Primary congenital glaucoma is the most common form of glaucoma in children.
Despite the rarity of this disease, it represents one of the major causes of irreversible blindness in children.
The aim of this study is to analyze the epidemiological and clinical aspects of the disease, and to report on our experience
surgical ; experience in the Casablanca Pediatric Ophthalmology Department.
Setting
This was a retrospective descriptive and analytical study of 327 cases (607 eyes) of congenital glaucoma operated on and monitored at the Pediatric Ophthalmology department of the 20 Aout 1953 Hospital of Casablanca, from January 2001 to January 2021.
Methods
The study involved 327 children (607 eyes), the mean age at diagnosis was 6.4 months (1 week to 15 years), the sex ratio was 1.6 with a slight male predominance of 51.8%,
Results
consanguinity was noted in 43.9% of cases, with 13.2% of similar cases in the family. The main reason for consultation
was dominated by megalocornea (75%), lacrimation (52.1%) and photophobia (40,9%). Glaucoma was bilateral in 87.1% of cases. Mean preoperative intraocular
was 21.3 mmhg. Corneal transparency was altered in 66% of eyes. The mean preoperative axial length was
22.3mm.Trabeculectomy combined with 5fluorouracil or mitomycin was performed in 97% of cases. This allowed normalization of ocular tone in 45% after the first surgery, in 72.2% after repeat surgery and in 82% after a second operation. For non-stabilized eyes, the average time to repeat surgery was 10 months.
Conclusions
Despite advances in microsurgery, the prognosis of congenital glaucoma remains guarded. The severity of this disease continues to draw attention.early diagnosis and immediate surgical intervention are capable of preventingthe onset of amblyopia and avoid blindness.
