Purpose

To evaluate the clinical and functional results of correction of myopia and myopic astigmatism using CLEAR technology based on the analysis of anatomical and topographic changes in the cornea, indicators of visual acuity and refraction in the pre and postoperative periods, and the condition of the posterior segment of the eye.

Setting

1252 CLEAR operations (626 patients).Gender distribution: 219 males (35%) and 407 females (65%).The age from 16 to 49 years (32.04±6.74). Sph from -0.25 to -9.0D(-3.76±1.48D), cyl from -0.0 to -2.25D(-0.53±0.34 D),VA in the distance without correction was 0.07±0.06, and VA with the best correction was from 0.8 to 1.0(0.99±0.01). The central thickness of the cornea from 487 to 634 microns.The WtW is from 10.9 to 13.4 mm. The Average curvature of the cornea was from 39.5 to 47.25D(43.39±1.32 D). 

Methods

Femtolaser lenticular correction of of myopia and myopic astigmatism on the platform of the LDV Z8 femtosecond laser ("Ziemer"). The follow-up period was from 4 months to 12 months. Of the additional examination methods, patients underwent optical coherence tomography on the Nidek RS– 3000 Advance AngioScan device (NIDEK, Japan) in order to evaluate the following indicators: vascular network density and blood flow index in the macular region of the retina and in the brain before surgery and 1 day after the CLEAR correction, as well as to determine the size of the premacular bag before surgery and 1 day later. 

Results

Visual acuity without correction for 1 day after surgery (1252 eyes) 0.93±0.2, sph -0.11±0.5D, cyl-0.4±0.5D; visual acuity without correction after 4-6 months (323 eyes) 1.05±0.24, sph -0.16±0.5D, cyl -0.4±0.5D; visual acuity without correction after 9-12 months (44 eyes) 1.04±0.2, sph -0.11± 0.4D, cyl-0.5±0.6D. Safety index 1.0, efficiency index 1.05. Percentage of intraoperative complications and in the early postoperative period less than 1%. 

Conclusions

Correction of myopia and myopic astigmatism using CLEAR technology is a predictable, safe, effective technology. In patients after performing vision correction using CLEAR technology, there are no statistically significant changes in the vascular network density and blood flow index in the macular region of the retina and in the ON, as well as the size of the premacular bag 1 day after surgery.
