Results Of 3-Year Eficcacy After I Stent Inject Combined With Cataract Surgery In Glaucoma Patients

Purpose

To evaluate the 3-year efficacy after iStent inject insertion combined with cataract surgery in glaucoma patients, in terms of intraocular pressure (IOP), reduction of drug administration and visual acuity (VA).

Setting

Hospital Juan Ramón Jiménez. Huelva. Spain

Methods

Twenty-seven eyes (16 open angle glaucoma, 10 hypertensive and 1 pigmentary glaucoma) were surgically treated by iStent inject implantation (Glaukos, Corporation, CA) combined with cataract surgery. Treatment efficacy was assessed by analyzing differences in mean IOP values before and up to 3 years after surgery, number of topical ocular medications administered and VA, at visits at 15 days, one and a half months, 3 months, 6 months, 1, 2 and 3 years.

Results

The average IOP values began to be significantly lower from one and a half months after surgery and were maintained until 3 years (p<0.01), with a maximum reduction of 24.8% at 6 months. The percentage reduction in the number of drugs was 64.1%, going from an average value of 1.44 drugs before surgery to 0.52 at 3 years (p<0.01). At 59.3% of the eyes were administered 2 or more hypotensive drugs before surgery while at 3 years the percentage was 11.1%. The percentage of eyes not treated with hypotensive drugs increased from 22.2% before surgery to 70.4% 3 years after surgery. The mean VA increases significantly from 0.33 to 0.84 decimal (p<0.01).

Conclusions

The insertion of iStent inject implants combined with cataract surgery is an effective option that is maintained for at least 3 years after surgery.
