Purpose

To report a rare ophthalmic manifestation of periampullary pancreatic tumor.

Setting

case report of periphral ulcerative keratitis (PUK)

Methods

We reported a rare case of 70 years old female patient with PUK as an ocular manifestation in the presence of underlying systemic solid organ malignancy. Ocular redness, pain, and photophopia are the major symptoms. Examination of right eye showed conjunctival injection, peripheral corneal infiltration which was treated by topical antibiotic, doxycycline and topical prepared medroxyprogesterone. Our provisional diagnosis was periphral ulcerative keratitis after ruling out infectious and autoimmune causes. During follow-up time, she presented with jaundice and recurrence of corneal symptoms. Laboratory results showed abnormal liver enzymes and amylase level. Histology of periampullary pancreatic tissue showed mucosal adenocarcinoma.

Results

The early diagnosis and appropriate management of PUK in patients with malignancies are vital to preserving sight and saving a life. This case highlights that, besides other infectious or autoimmune causes, PUK may be associated with life-threatening systemic solid pancreatic neoplasm.

Conclusions

PUK can be presenting manifestation of solid neoplasm that should be considered among differential diagnosis especially in elderly patient.
