Evaluation Of Ocular Surface Parameters In Patients With Obstructive Sleep Apnea Syndrome

Purpose
To investigate the ocular surface characteristics in patients diagnosed with obstructive sleep apnea syndrome (OSAS) according to the severity stage of the disorder.
Setting
Marmara University, School of Medicine, Department of Ophthalmology
Methods
Fifty-one patients with OSAS were included in this study. The severity of OSAS was determined by Apnea Hypopnea Index (AHI). Ocular surface parameters were evaluated as follows: non-invasive tear break-up time (NI-TBUT), Marx’ line, meibomian gland loss (MGL), and the Dry Eye Questionnaire (DEQ-5). Eyelid laxity was measured with medial canthal laxity (MCL) and tarsal exposure (TE).
Results
There were 21 patients in mild and 30 patients in severe OSAS group. NI-TBUT was lower in severe group(9.53±5.35)compared to mild group(12.28±5.66)(p=0.017).The Marx line score was higher in the severe group(3.18±1.99) than the mild group(2.23 ± 2.42) (p = 0.033). MGL was higher in severe group(51.0.2±13.98)compare to the mild group(39.27 ± 12.93)(p<0.001). The DEQ-5 was similar between the groups(p=0.867).The MCL was grade 1 or higher in 54.8% of the mild group and 41.7% of the severe group.TE was grade 1 or higher in 50.0% of the mild group and 61.7% of the severe group.Grade distribution in MCL and TE was similar between groups(p= 0.343, p=0.092).A significant correlation was observed between MCL(r=0.60, p<0.001)and TE(r= 0.52, p<0.001).
Conclusions
While the severity of the disease was not associated with the eyelid laxity, it had a negative impact on parameters related to evaporative dry eye.
