Innovative Technique To Treat Keratoconus

Purpose

To analyze 10-year- outcomes of a femtosecond-assisted -technique to treat keratoconus and corneal ectasia.

Setting

Barranquilla, Colombia and Buenos Aires, Argentina

Methods

Three hundred and eight of moderate to severe keratoconic cases which underwent Corneal Remodeling were analyzed. Regarding the procedure, anesthetic drops were instilled.  A platform designed for femtosecond laser was used to perform the ablation: an 8 mm diameter-270 degree to 360 degree- keratectomy (arc lenght depended on the topographical profile in each case). Once ablation was completed, the edges of the resection were sutured with interrupted stitches.

Results

Age of the population was 31.65 ± 18.55 years-old. At 10th year,100% of the cases changed preoperative Against-the-Rule or Oblique Astigmatism to postoperative With-the-Rule Astigmatism. Corrected Distance Visual Acuity (CDVA) changed from 0.32± 0.26 to 0.67± 0.31. Cases in which excimer laser was performed Uncorrected Distance Visual Acuoty (UDVA) was 20/40 or better. All of the cases gained at least 1 line of CDVA; 51% of the cases gained 4 to 6 lines of CDVA. Postoperative Coma Aberration decreased up to 78%. Also we analyzed the hysteresis before and after performing the procedure.

Conclusions

Corneal Remodeling is a safe technique. Produces a new keratometry values and decreases optical aberrations. It allows surgeons to perform any complementary refractive procedure.
