Corneal Cross Linking & Insuline Drops: Results Of A Series Of Cases.

Purpose
To describe the results of a series of cases of patients with progressive keratoconus who were treated with Epi off corneal cross linking in association with insulina eye drops in their postoperative treatment.
Setting
All patients were treated at Cornea Department of Zaldivar Institute in Mendoza, Argentina.
Methods
20 eyes of patients with progressive keratoconus were treated with Epi-off accelerated CXL, in all cases the traditional postoperative treatment (prednisolone acetate, antibiotics and eye drops) was used in association with insuline eye drops (1UI/ML).
All patients were monitored on 3 consecutive postoperative days after the CXL. Ocular pain, photofobia, conjuntival hyperemia and corneal healing were evaluated in the inmediate postop days, and confocal microscopu in the 1 and 3 months controls.
Results
All eyes had completed corneal epithelial defect by 72 hs after the surgery, no complications were described in the postoperative evaluations.
Ocular pain and objetive evaluation scores reveal a reduction in the subjective pain symptoms as well as an increase in the speed of epitheliazation in this series of cases. Confocal microscopy in controls reveals and increase in the speed of regeneration of the corneal subbasal nerve plexus.
Conclusions
The use of topical insuline drops in the postoperative treatment of Epi off CXL procedure accelerate corneal reepithelizacion, improve stromal healing, increase the corneal subbassal nerve plexus and reduce ocular symptoms after the surgery.
