Purpose

To study the results of Triva Trifocal posterior chamber IOL in patients with age related cataracts. 

Setting

The study was conducted over the period of 2022-2024 in a clinical setting where 40 patients underwent cataract surgery and received Triva Trifocal posterior chamber IOLs.

Methods

Forty otherwise healthy patients with age-related cataracts were selected for Triva IOL implantation. Out of 40 patients only one 62 yo female had history of bilateral Photorefractive Keratectomy 10 years ago for mild myopia. The age range was between 49 yo and 66 yo with the mean age of 56 yo. The uncorrected visual acuity range before the surgery was 20/100 – 20/30. In all the cases the IOL calculation was performed with Alcon Verion Image guided system by Holladay 2 formula. Before the surgery each patient underwent thorough eye exam including visual acuity test, intraocular pressure measurement, slit lamp fundus exam, B-Scan, A-scan, autorefractometry, Oculyzer. All surgeries were performed by the same surgeon and were uneventful.

Results

In all 40 eyes, 20/20 distance and J2 near vision were achieved within 1-3 days post-surgery. However, a 62-year-old female with bilateral PRK history presented unique challenges. Pre-surgery visual acuity was 20/200, and IOL calculation revealed the same number of IOLs (20.5D) with the mean not significant residual refractive error of sph(+)0.21 cyl(-)0.33 ax134. The first eye (OD) achieved 20/80 the next day with BCVA of 20/20. Three months later being stable she underwent LASIK and was 20/20 the next day. Despite this, the patient opted for the same surgery for OS, saying she will do LASIK again if needed. Remarkably, OS achieved 20/20 distance and J2 near vision the next day. Both eyes maintained stability during the 11-month follow-up.

Conclusions

The Triva Trifocal posterior chamber IOL proves effective in addressing presbyopia in age-related cataract patients. Nonetheless, caution is warranted in individuals with a history of corneal refractive surgery, as unexpected outcomes may occur․
