Comparison Of Clinical Outcomes Between Two Different Types Of Trifocal Iols In High Hyperopic Eyes.

Purpose

To compare the visual outcomes and patient satisfaction in high hyperopic eyes after the implantation of two different types of diffractive trifocal IOLs.

Setting

The Prospective non-randomised comparative study in which sequential bilateral cataract surgery by phacoemulsification technique was performed by a Single Surgeon at Aastha Hospital, Anand, Gujarat, India between August 22 to April 23. All patients wanted spectacle freedom and, advantages and disadvantages of both the IOLs were informed. The study adhered to the Declaration of Helsinki and was approved by the local ethics committee(1132/ac/07/CPCSEA).

Methods

This study was conducted in 8 eyes of 5 high hyperopic patients(axial length<22.50mm or IOL power>25.0D) who underwent cataract surgery with the implantation of Tecnis Synergy (ZFV00) or AT LISA Tri toric (939MP) between Aug 22 to April 23. Post-op examination at one month included uncorrected visual acuity (UCVA) at 3M, uncorrected intermediate visual acuity (UICA) at 80 cm, UICA at 60 cm, uncorrected near visual acuity (UNCA) at40 cm, contrast sensitivity and a questionnaire on quality of vision, optical phenomena and spectacle independence.

The inclusion criteria were age of patients between 54 to 67 years, axial length (AL)< 22.50 mm, IOL power>25.0D, Corneal astigmatism > 1.0D, scotopic pupil size ranging from 3 to 6 mm

Results

The two groups were similar in baseline demographics, pre-operative visual, biometric data, and surgical parameters.The standard visual acuity testing of ZFV00 and 939MP are UCVA at 3m; 0.24± 0.07 vs 0.21 ± 0.06 logMAR,p = 0.20; UIVA (80 cms);0.00± 0.11 vs 0.20 ± 0.08 logMAR,p=0.03 UIVA (60 cms);0.05± 0.06 vs 0.23± 0.13 logMAR, p=0.03  and UNVA at 40 cm; 0.15± 0.06 vs0.23 ± 0.13 logMAR, p=0.23 respectively.The results revealed that patients who received 939MP exhibited slightly better-uncorrected distance vision, while those with ZFV00 had superior Intermediate and near vision. Patient satisfaction was generally high, with minor variations between the two IOL types. Contrast sensitivity results were similar for both groups.

Conclusions

AT LISA Tri toric 939MP was associated with good far vision, while Tecnis Synergy ZFV00 with better intermediate & near vision. Both IOLs showed favourable visual outcomes & patient satisfaction in hyperopic patients.
