Purpose
To assess subjective depth of focus (DoFs), refractive outcomes, and objective depth of focus (DoFo) according to through-focus augmented VSOTF in eyes with irregular astigmatisms that underwent cataract surgery with a small aperture IC-8 Apthera IOL using ray-tracing wavefront analysis.
Setting
Medical University of South Carolina- Storm Eye Institute, Charleston, SC.
Methods
This is a prospective observational study involving eyes with irregular astigmatism that underwent cataract surgery with IC-8 Apthera IOL (B&L) implantation. Post-laser vision correction surgery, radial keratotomy, and ectasic eyes were included. Lower- (LOAs) and higher-order aberrations (HOAs) at 6-, 4-, and 2 mm corneal plane aperture sizes were measured preoperatively and postoperative using Pentacam Wave (Oculus).Postoperative assessment included refractive outcomes (UCVA, DCVA, DCIVA, DCNVA), subjective depth of focus (DoFs) curves, and objective DoFo using visual Strehl optical transfer function (VSOTF) on ray tracing (Tracey technologies) aberrometer.
Results
Mean post-operative UDVA was 0.12 logMAR ± 0.12 (20/26), CDVA was 0.03 logMAR ± 0.05 (20/21.5), UNVA was -0.11 logMAR ± 0.16 (20/15.5) and DCNVA was 0.03 logMAR ± 0.05 (20/21). Postoperatively, this was consistent with the measured ocular RMS HOAs through the small aperture IOL (0.09 ± 0.11 µm). The DoFs ranged between 3.00D to 5.00 D yield a mean value of 4.16 ± 0.74 D for functional vision of 20/40 or better. Objective DoFo based on through-focus augmented VSOTF at 50% threshold was 3.91 ± 0.20 D (range 3.29 D to 4.1 D) and at 30% threshold was 5.13 ± 0.22 D (range 4.85 D to 5.60 D). Conclusions
After IC-8 Apthera IOL implantation, ocular higher-order aberrations were within the normal range for healthy eyes. The small aperture optics provide a wide depth of focus, resulting in satisfactory visual acuity for near, intermediate, and distance vision in individuals with irregular corneas. Correlations between objective DoFo. using VSOTF on ray tracing aberrometer and clinical DoFs were examined. Using the small aperture IOL clinically derived mean DoF is slightly more comparable to the mean objected DoF at 50% threshold to the VSOTF values.
