Comparison Of Clinical Experiences With A Pure Refractive And A Novel Extended Depth Of Focus Intraocular Lens Based On Wavefront Linking Technology

Purpose

During the last couple of years, numerousintraocular lenses (IOLs) designed to provide continuousfunctional vision for far and intermediate distances haveentered the market. The IOL manufacturers utilize variousoptical principles and technologies to meet the ever-increasingdemand of patients for visual comfort. The currentinvestigation compares our clinical experiences collectedduring the first postoperative year after the binocularimplantation of two extended-depth-of-focus (EDOF) lenses, the LuxSmart IOL by Bausch + Lomb, and the Elon 877PEY lens by Medicontur.

Methods

Cataract patients without remarkable preoperativecorneal astigmatism were included in our investigation. Eachof them was binocularly implanted with one of the IOLs in focus. Postoperative visits were scheduled for 1 day, 1, 3, and 6 months and 12 months following surgery. Monocular and binocular visual acuity measurements (uncorrected and distance-corrected) were performed using ETDRS and Jaegervisual acuity charts during each visit. Distance-correctedvisual acuity and contrast sensitivity defocus curves wereregistered using the Multifocal Lens Analyzer application(Qvision, Spain). Patients were asked about the sensation of dysphotopsia. Visual functions and spectacle use wereassessed in both study groups.

Results

Both investigated IOLs provided a refractivecorrection of high accuracy and predictability. Both lensesrestored distance vision efficiently, however intermediate and near vision in the Elon group were found to be superior. The binocular visual acuity defocus curves were above 0.2 logMAR along a 2.4 D defocus range with the Elon, and 1.5 D with LuxSmart. Remarkable differences could be observedboth in the intermediate and near range (from -1.50 to -3.50 D). All patients were satisfied with the visual outcomes, although LuxSmart patients were more prone to wearspectacles for near-vision tasks and from time to time also forarm-length activities.

Conclusions

Our experiences suggest that both examinedintraocular lenses provide predictable refractive outcomes and good distance vision, however, the Elon 877PEY model basedon wavefront linking contributes to a wider range of excellentquality vision, than the LuxSmart IOL. ELON patientsreported the perception of dysphotopic phenomena will less frequency and intensity and found the performance of theirdaily tasks less difficult, than those receiving the other lens. Based on our experience and on our patients’ feedback webelieve, that while LuxSmart is a good enhanced monofocallens, the Elon IOL grants a real EDOF vision of high quality.
