Comparison Of Intra-Operative Efficiencies And Surgical Outcomes Between Legion (Torsional Ultrasound) Versus Signature Pro (Transverse Mode)

Purpose
To compare the intraoperative efficiency and surgical outcome of 2 phacoemulsification technologies: The Legion torsional (Ozil technology) and Signature pro transversal (Ellips FX).
Setting
Single centre, prospective randomized comparative study conducted at Narayana Nethralaya, Bangalore, India.
Methods
120 eyes of 112 patients with senile cataract grade 2&3 on Lens Opacities Classification System III(LOCS) were randomized in 2 groups of 60 each. Torsional(Legion, Alcon Laboratories, Inc,USA) or transversal(Whitestar SignaturePro, Johnsons&Johnsons, Inc,USA) phacoemulsification was performed by same surgeon. Biometry,specular microscopy, pachymetry, best-corrected visual acuity were done preoperatively & postoperatively on day1,30,& 90. Intraoperative parameters like Incision Size, viscoelastics used, Nucleus Management technique, type of intraocular lens, Power, Vacuum, Aspiration Flow Rate, mean case time, ultrasound time, Phaco probe time, wound hydration time were noted. Surgical video was assessed by an independent masked evaluator.
Results
The ultrasound time and the mean case time were 47.12% and 5.15% lesser for the Ozil technology as compared to Ellips FX. Ozil technology had notably less chatter, followability and the ease of quadrant removal. The visual outcomes were not statistically significant between the 2 groups. The difference between pre-operative pachymetry compared to that on POD 1 & 30, and pre-operative endothelial cell density compared to that on POD 1, 30, & 90 were also not statistically significant in both the groups.
Conclusions
The power delivery for Legion (Torsional Ultrasound) proved to be more efficient than Signature Pro (Transverse mode). However, the postoperative visual outcomes between the 2 modes of power delivery were found to be comparable.
