Anterior Chamber Fibrin Reaction After Sing Imt Telescope Implantation

Purpose

To report the management of anterior chamber fibrin reaction after SING- IMT telescope implantation

Setting

Palagi Hospital ASL Toscana Centro Firenze, Italy

Methods

Case report

Results

An 83-year-old-woman developed a marked anterior chamber fibrin reaction, with conjunctival hyperemia and chemosis, the first day after lens extraction and smaller-incision new-generation implantable miniature telescope (SING IMT) implantation for late-stage age-related macular degeneration. Anterior segment OCT showed a dense fibrin clot over the telescope with some strands attached to the corneal endothelium. A subtenon injection of 420 mg Triamcinolone acetonide was performed. After 2 -days, there was a reduction in conjunctival hyperemia and chemosis, along with partial resolution of anterior chamber fibrin

Conclusions

Anterior chamber fibrin reaction, though rare, can manifest following implantation procedures such as SING IMT for late-stage age-related macular degeneration. Prompt recognition and intervention, such as subtenon injection of Triamcinolone acetonide, have shown efficacy in addressing  symptoms and resolving fibrin accumulation
