Purpose

To report an unusual pseudo-endophtalmitis reaction after an intravitreal injection (IVI) of triamcinolone acetonide.

Setting

We report two interesting cases of pseudo-endophthalmitis, following intra-vitreal injection of triamcinolone acetonide, for cystoid macular edema in Irvine-Gass syndrome. Received at the Ophtalmology A department, hospital of specialties, Rabat, Morocco

Methods

Two patients are reported with a pseudo-endophthalmitis following an intravitreal injection of triamcinolone acetonide for post-operative Cystoid macular edema. They were pseudophakic and had a diagnosis of Irvine-Gass syndrome. The first patient had a posterior capsular rupture and the second with previous history of combined surgery (for post-traumatic cataract and retinal detachment) had a suspected zonular defect.

Results

They both received intravitreal Triamcinolone Acetonide injection after medical topical treatment’s failure. Two days following the injection, they complained of painless vision loss and presented with a clinical picture of pseudo-hypopyon. The two cases resolved spontaneously around 1 month.

Conclusions

Pseudophakia with posterior capsular rupture, zonular defect and cystoid macular edema due to Irvine-Gass syndrome are risk factors of pseudo-endophthalmitis following IVI triamcinolone.
