Managment Of Fuchs Uveitic Cataract - About One Case :

Purpose

The particularity of Fuchs uveitis cataract surgery from non uveitis ones. Showcasing the importance of perioperative management to prevent and decrease the complications rates either during surgery or postoperatively resulting in the best prognosis possible.

Setting

Reporting a  case of a patient presenting unilateral fuchs uveitis complicated with mature cataract. Received at the Ophtalmology A Service, Department of Ophthalmology , Specialty Hospital, Faculty of Medicine of Rabat, University of Mohamed V , Morocco .

Methods

A 50 year old male patient, with history of anterior uveitis attack in the context of Fuchs syndrome, presents a visual acuity of left eye at hand motion related to mature cataract. The ocular quiescence has been maintained for 6 months allowing the cataract surgery to proceed. For preoperative optimization , the patient was put under prednisone 1mg/kg/J for 1 week prior. 

Results

The procedure was performed under topical anesthesia, after uncomplicated standard phacoemulsification surgery, in-the-bag-IOL implantation.The patient received intravenous methylprednisolone after surgery, and put under topical association of dexamethasone and neomycin every 2 hours for 48 hours then at decreasing doses for 6–8 weeks.Post-operative results after 3 months follow up are very satisfying even with the presence of most common complications that are hard to avoid.

Conclusions

Phacoemulsification in patients with Fuchs uveitis does not currently present any difficulties compared with normal senile cataract. No major complications occurred .The cataract surgery is safe with very good visual outcome.
