Management Of Aqueous Humor Leakage After Cataract Surgery

Purpose

Description of aqueous humor lakage and managment options of this postsurgical complication. 

Setting

Cataract is the leading cause of blindness in the world, the majority related to age and therefore there is no way to prevent its appearance. For its treatment, the most commonly used technique is phacoemulsification and to carry it out initially, one or two paracenteses  and a main incision Sutureless clear corneal incisions (CCIs) have become the standar. However, there is a concern that leaving incisions without definitive closure may increase the risk for wound leaks

Methods

Scientific literature search and clinical experience. 

Results

Sutureless clear corneal incisions have become the standard of care when performing routine cataract surgery due to multiple advantages. However, there is a concern that leaving incisions without definitive closure may increase the risk for wound leaks

Seidel testing is the most common method used to test for leaks. During this test, fluorescein is applied to the incision and the surgeon looks for fluid egress.

We can use different techniques to promote the correct closure of the incision and stop the outflow of aqueous humor, such as: Occlusion, therapeutic contact lenses, different suturing techniques, wound closure products, conjuntival flap surgery that promote correct apposition of the lips of the incision to minimize leakage.

Conclusions

Definitive wound closure is critical to the overall success of cataract surgery. Current standards of care, such as stromal hydration and sutures, do not appear to provide sufficient wound integrity to guarantee a definitive seal. It is important to know alternatives to take care of wound leaks due to avoid other severe complications.
