Purpose

The multifocal intraocular lens (IOL) provides a wide bright field of vision due to its IOL design. On the other hand, we have experienced cases with practical intermediate vision even when a monofocal IOL is used for correct vision. However, there are few reports on the intermediate visual acuity of monovision IOLs. In this study, we examined the intermediate visual acuity of the Clareon® (Alcon) monofocal IOL.

Setting

Study design; Prospective, non-interventional, single arm study

Primary endpoints:

  • Binocular uncorrected visual acuity (66cm) at 3 month

Inclusion criteria

  • Binocular Cataract patients over 20 years old at time of informed consent.
  • Bilateral cataract patients scheduled to insert Clareon/ Clareon Toric in both eyes with emmetropia

Exclusion criteria

  • Any disease that affects visual acuity other than cataract
  • Corneal irregular astigmatism
  • Previous intraocular or refractive surgery

Methods

Thirty patients (mean age 69 years) underwent cataract surgery alone at Sugita Eye Hospital. All patients had Clareon® or Clareon® Toric inserted for binocular correct vision. Naked eye and corrected visual acuity were measured at 3 months postoperatively at distance (5 m) and intermediate distances (80 cm and 66 cm). Starting at a spherical power of +1.50 D and using the refraction at which best distance vision was obtained as the zero base point, binocular open eye visual acuity was measured at each additional power (+1.50, +1.00, +0.50, +0.25, 0, -0.25, -0.50, -1.00, -1.50, -2.00) and a Clareon® depth of focus curve The study was conducted as a prospective, single-comparison study.

Results

The binocular naked eye visual acuity (logMAR) were -0.11±0.06 (5 m), 0.12±0.09 (80 cm), 0.17±0.11 (66 cm), and the binocular distance-corrected visual acuity (logMAR) were -0.14±0.05 (5 m), 0.12±0.10 (80 cm), 0.15±0.11 (66 cm). The depth-of-focus curves (logMAR) were +1.50D(0.33), +1.00D(0.13), +0.50D(-0.07), +0.25D(- 0.12), 0D(-0.14), -0.25D(-0.13),  -0.50D(-0.08), -1.00D(0.04),-1.50D(0.17), -2.00D(0.37).

Conclusions

The monofocal IOL, Clareon ®, may have good distance vision and practical intermediate vision.
