Confocal Microscopy In Crohn’S Disease Keratopathy

Purpose

To report biomicroscopy and confocal microscopy findings of a patient with Crohn’s disease associated keratopathy.

Results

We report a case of a 50 year old male diagnosed of Crohn’s disease and subepithelial, grey corneal infiltrates of granular appearance. These were located inferiorly 3 mm inside the corneoscleral limbus. The central cornea remained clear.

Confocal microscopy showed hypereflective intra and intercellular deposits in the basal cell layer of the corneal epithelium and scattered plaques of diffused, irregular borders and at the level of the basement membrane.

Conclusions

Subepithelial nodular- granular opacities, linear formations with an epithelial basement membrane dystrophy-like appearance and subepitelial basement membrane scarring in the paracentral area scarring sparing the central cornea should warrant a medical examination for possible Crohn s disease.

Confocal microscopy may provide additional clues to the diagnosis of Crohn s keratopathy.
