Comparison Of Posterior Corneal Curvature Measurements By Different Tomographers

Purpose

The primary purpose of this study was to report and compare the posterior keratometry (PK) values obtained with 3 modern instruments using optical coherence tomography (OCT), namely the IOL Master 700, the Anterion (Heidelberg, Germany) and the MS-39 (CSO, Italy), and one instrument based on Scheimpflug imaging (Pentacam AXL Wave, Germany). Secondarily, we aimed to compare the anterior-to-posterior ratio (A/P ratio) and total corneal power (TCP).

Setting

Studio Oculistico d’Azeglio, Bologna, Italy

Methods

Eighty-four eyes of 84 patients who undergoing cataract surgery were enrolled. The PK, A/P ratio and TCP obtained by the 4 instruments were compared.
Repeated measures analysis of variance (ANOVA) or Friedman’s test were used to compare normally and not normally distributed data, respectively.

Results

The difference among the PK of the 4 devices was statistically significant (p <0.0001), with the IOLMaster 700 providing a mean lower power (-5.76±0.24 D) with respect to the Anterion (-6.06±0.24 D), MS-39 (-6.16±0.38 D) and Pentacam AXL Wave (-6.25±0.27 D). The A/P ratio was significantly lower (p <0.0001) with the IOL Master 700 (1.12±0.02) than with the Anterion (1.19 ±0.02), MS-39 (1.20±0.03) and Pentacam AXL Wave (1.22 ±0.03). The difference between TCP and Kave was statistically significant for all instruments but the IOLMaster 700.

Conclusions

The 4 instruments provided different PK measurements, with the IOL Master 700 showing the highest differences with respect to the others. These measurements cannot be considered interchangeable and should be considered by calculators that use PK to calculate the IOL power.
