Purpose
To show that Small incision lenticule extraction(SMILE) is a viable procedure to treat myopia and astigmatism in cases of primary megalocornea
Setting
Eye department of the international eye centre ,Mumbai
Report of case
STUDY DESIGN-This is an isolated case report of SMILE on primary megalocornea for moderate myopia PURPOSE-To show that Lenticule extraction is a viable alternative with no side effects in a case of Myopia with Primary Megalocornea . METHODS-A 31-years-old male ,6/6 with -2.50 D Sph RE and 6/6 in his left eye with-1.75 D sph/ -2.25 D cyl x 155 with bilateral megalocornea (wtw distance 14.8 mm), with a clear cornea and a pachymetry of 413 and 401 microns in the right and left eye, respectively presented for possible refractive surgery.Informed consent was obtained and the institutes ethical committe approved it.TheVisuMax500kHz femtosecond laser system (Carl Zeiss Meditec, Jena, Germany) was used. The SMILE procedure was performed ,first the femtosecond laser created the lenticule which was then extracted by the chief author .The cap was 120 microns with a diameter of 7.7 mm and an optical zone of 6.5mm
RESULTS -The patient was 6/6 ,N6 two days post procedure with no pain .At four years follow up the cornea and refraction are stable . CONCLUSION-SMILE is a viable solution for myopia and astigmatism in primary megalocornea
Conclusion/Take home message
- Literature review shows that PRK or any other refractive surgery has never been attempted in a primary Megalocornea .
- We can positively conclude that -SMILE FOR MYOPIA AND ASTIGMATISM IN PRIMARY MEGALOCORNEA HAS PROVED TO BE A VIABLE AND SAFE PROCEDURE.
