Purpose

EVALUATE THE EFFECT OF TOPICAL LOSARTAN IN CORNEAL SCARRING FIBROSIS MEDIATED BY THE DEVELOPMENT OF MYOFIBROBLASTS AFTER TRAUMATIC INJURY, MICROBIAL INFECTIONS, SCARRING DISEASES AND SOME CORNEAL SURGERIES.

Setting

A CASE OF CORNEAL SCARRING DUE TO FUNGAL ULCER AND ANOTHER ONE WITH CORNEAL FIBROSIS AFTER CORNEAL RING EXTRUSION AT CARLOS ANDRADE MARIN HOSPITAL (QUITO ECUADOR) ARE PRESENTED 

Report of case

  1. A 38 YEARS OLD MALE PATIENT PRESENTED MICOTIC ULCER AFTER FUSARIUM INFECTION IN THE LEFT EYE. UNCORRECTED DISTANCE VISUAL ACUITY (UDVA) WAS 20/200 AND CORRECTED DISTANCE VISUAL ACUITY WAS 20/100 THE PATIENT STARTED USE TOPICAL LOSARTAN 3.6MG/ML FOUR TIMES FOR DAY. AFTER ONE MONTH WE GOT UNCORRECTED DISTANCE VISUAL ACUITY (UDVA) WAS 20/80 AND CORRECTED DISTANCE VISUAL ACUITY WAS 20/40 AND A SIGNIFICANT REDUCTION OF CORNEAL HAZE WAS OBSERVED AT THE SLIT LAMP AND USING ANTERIOR SEGMENT OPTICAL COHERENCE TOMOGRAPHY.
  2. A 34 YEARS OLD MALE PATIENT PRESENTED CORNEAL FIBROSIS AFTER CORNEAL RING EXTRUSION INFECTION IN THE LEFT EYE. UNCORRECTED DISTANCE VISUAL ACUITY (UDVA) WAS 20/500 AND CORRECTED DISTANCE VISUAL ACUITY WAS 20/70 THE PATIENT STARTED USE TOPICAL LOSARTAN 3.6MG/ML FOUR TIMES FOR DAY. AFTER TWO MONTHS WE GOT UNCORRECTED DISTANCE VISUAL ACUITY (UDVA) WAS 20/400 AND CORRECTED DISTANCE VISUAL ACUITY WAS 20/40 AND A SIGNIFICANT REDUCTION OF CORNEAL HAZE WAS OBSERVED AT THE SLIT LAMP AND USING ANTERIOR SEGMENT OPTICAL COHERENCE TOMOGRAPHY.

Conclusion/Take home message

BY ANALYZING THE EFFECT OF THIS MEDICATION WE KNOW THAT THERE ARE TWO GROWTH FACTORS: TRANSFORMING GROWTH FACTOR (TGF) BETA-1 AND TGF BETA-2. WHICH INFLUENCE THE DEVELOPMENT OF MYOFIBROBLASTS. LOSARTAN CONSTITUTES A TGF BETA SIGNALING BARRIER BECAUSE IT INHIBITS AN EXTRACELLULAR SIGNAL TRANSDUCTION MOLECULE SIGNAL-REGULATED KINASE (ERK).
