Purpose
To describe a case of successful DALK in a patient with previous fungal ulcer
Setting
The case of a male patient with corneal leukoma after fungal ulcer due to Candida Labrata who underwent DALK without reactivation of previous infection at Carlos Andrade Marin Hospital (Quito Ecuador)
Report of case
A 15-year-old patient who presented with corneal ulcer due to candida labrata with good response to fluconazole eye drops. The corneal leukoma was predescemetic, so a deep anterior lamellar keratoplasty was performed using the manual Melles technique. Each dissected corneal stromal layer was analyzed in an anatomy pathology laboratory and bacterial and fungal cultures were taken for ensure an infection-free bed. All results were negative for fungi.
Corneal leukoma after fungal ulcer was successfully resolved, postoperative management with low doses of corticosteroid while waiting for the result of the culture did not influence the corneal transparency obtained after DALK
Conclusion/Take home message
Deep anterior lamellar keratoplasty may be a valid alternative in inactive corneal fungal infections considering the advantages of this technique in young people by preserving their corneal endothelium.
