Repetibility And Agreement Of Corneal Epithelium Thickness In Patients Undergoing Photorrefractive Keratectomy (Prk)

Purpose
To compare the repeatability and agreement between two SA-OCT devices (MS39® and Anterion®) in the measurement of corneal epithelial thickness in patients undergoing PRK surgery.
Setting
Miranza Begitek. Donostia – San Sebastián, Spain.
Methods
Patients undergoing PRK surgery were included. Three measurements were performed at each visit (preoperative, 1 week, and 3 months after surgery) with the MS-39® (CSO, Italy) and the Anterion® (Heidelberg, Germany). Epithelium thickness was measured in the central cornea and in several mid-peryphery sectors in all quadrants. Maximum and minimum values were also recorded. The intrasubject standard deviation (Sw), the coefficient of variability (CoV), the repeatability coefficient (R), the precision, the intraclass correlation (ICC), the limits of agreement (LoA) at 95% and the Bland-Altman plots were analyzed.
Results
98 eyes of 98 patients were assessed. 48% were women. Mean age of the patients was 32.83 ± 6.87 years, and the preoperative spherical equivalent was -2.47 ± 1.35 diopters. The Sw of central area with MS-39® was 0.53 (pre-op), 2.22 (7 days post-op), and 1.13 (3-month post-op) and with Anterion® was 0.56 (pre-op), 1.64 (7 days post-op) and 0.61 (3-month post-op). The ICC of 0-3mm area with MS-39® was 0.973 (pre-op), 0.846 (7 days post-op), and 0.920 (3-month post-op) and with Anterion® at 0-2mm area was 0.968 (pre-op), 0.969 (7 days post-op) and 0.971 (3-month post-op). There were no statistically significant differences (p=0.287) in maximum epithelium thickness at 3 months between MS-39® and Anterion® (61.43 ± 4.07 μm vs. 61.48 ± 5.27 μm).
Conclusions
Both devices show good repeatability preoperatively and 3 months after surgery, however, one week after surgery the repeatability of epithelial thickness decreases in all areas with both instruments. Anterion recovers preoperative repeatability level quicker than MS39. The maximum, minimum and central epithelial thickness values are not interchangeable between both devices. MS-39 epithelial thickness is slightly thicker.
