Patient Reported Outcome Measures Related To Quality Of Life After Laser Scleral Microporation For Presbyopia

Purpose
To report improvements in Patient Reported Outcome Measures (PROMs) of satisfaction of near activities after undergoing bilateral Laser Scleral Microporation (LSM) for the recovery of Dynamic Range of Focus (DROF) and the effect of Health Related Quality of Life(HRQoL). Results reported up to 24 months
Setting
Asian Eye Institute, Philippines
Methods
Prospective pilot clinical study of 47 presbyopic patients (94 eyes) with a mean age of 52.8 ± 4.0 years. Subjects demonstrated a loss of DROF defined as DCNVA at 40 cm ≥ 20/50 and requiring a reading add ≥ +1.50 D; with a UDVA of 20/40 or better. Emmetropic subjects underwent bilateral LSM therapy to address presbyopia. Patient reported outcomes were assessed using the Near Activity Visual Questionnaire (NAVQ), a standardized questionnaire, to measure satisfaction with near vision at baseline and post-LSM at one month, 3, 6, 12, 18, and 24 months. Patient Rausch scores were divided into three functional vision categories of near, intermediate, and special conditions. Rausch scores were evaluated to understand effects on HRQoL
Results
PROM results found 74% of subjects reporting moderate to complete satisfaction with near vision at 12 months. Significant improvements in near functional vision tasks (items 1-3), intermediate functional visual tasks (items 4-7) and functional ADLs for special circumstances (items 8-10) were demonstrated after LSM at 12 and 24 months. The emotional weight of functional limitations varies with lifestyle and gender.
Conclusions
Patient reported outcome using the NAVQ demonstrated the positive impact of LSM therapy on satisfaction with near visual function. Patient satisfaction was not directly tied to the amount of reading ability. The general trend for improvement for near and intermediate functional tasks was positive after LSM. Special task satisfaction was more related to lifestyle. NAVQ successfully demonstrates the quality of life improved after LSM.
