Purpose
This study aimed to investigate in the changes in the superficial retinal capillary plexus (SCP), deep retinal capillary plexus (DCP), choriocapillaris (CC), and central retinal thickness (CRT) in individuals with high myopia following V4c implantable collamer lens (ICL V4c) implantation utilizing optical coherence tomography angiography (OCTA).

Methods
A retrospective study was conducted at Tianjin Aier Eye Hospital from October 2022 to May 2023, focusing on patients with high myopia who underwent ICL V4c implantation. The study included 30 participants (14 males, 16 females). analysis included uncorrected visual acuity (UDVA), intraocular pressure (IOP), spherical equivalent (SE), and OCTA examinations performed before surgery and at 1 week, 1 month, and 3 months post-surgery. The OCTA examination measured the foveal avascular zone (FAZ), vascular density (VD) of SCP and DCP, blood flow density (BD) of CC, and CRT. These measurements were obtained using the instrument's built-in software. Statistical analysis used SPSS 27.0 with GraphPad Prism 9.5 for visual representation
Results
UDVA and SE at 1 week, 1 month, and 3 months post-surgery significantly improved. IOP fluctuated post-ICL implantation, with significant changes during follow-up period . FAZ and CRT remained stable, while VD decreased notably in the nasal region (S3) of superficial retinas (3mm diameter) postoperatively. In superficial retinas with a diameter of 6 mm, VD in the nasal (S7) and temporal (S9) regions showed significant changes overall postoperatively. Moreover, the DCP and other branches of SCP exhibited no alteration before or after the surgery. Notably, there was an obvious overall increase in blood flow density (2 mm diameter) postoperatively.
Conclusions
In this study suggested that ICL V4c implantation demonstrate excellent postoperative visual acuity and quality for high myopia patients with. However, attention should be given to the effects of surgery on retinal and choroidal microcirculation.
