Long-Term Outcomes Of Phakic Posterior Chamber Intraocular Lens Implantation For Moderate To High Myopia And Astigmatism

Purpose
To assess the 5-year clinical outcomes of an implantable collamer lens (ICL) with central aquaport (EVO-ICL) for myopia and astigmatism
Setting
NeoVize Eye Clinic Brno, Czech Republic
Methods
The retrospective study comprised 66 eyes of 35 patients with spherical equivalents of -6.55±4.33diopters (D) who underwent EVO-ICL implantation. Variables analyzed were preoperative, 1-month, 1-year, 5-year, and last follow-up uncorrected distance (UDVA) and corrected distance visual acuity (CDVA), manifest refraction, spherical equivalent (SE), endothelial cell count (ECC), vault, and intraocular pressure (IOP).
Results
66 eyes (35 patients) were analyzed; the mean follow-up was 5.22 ± 1.30 years. The mean preoperative SE was -9.06 ± 3.40 D, and the mean preoperative cylinder -1.55 ± 1.55 D. At the last follow-up, efficacy and safety indices were 0.79 and 1.12, respectively. At five years, 83 and 93% of eyes were within ± 0.5 and ± 1.0D of the targeted correction, respectively. 31% eyes had ≤1.00 D of postoperative astigmatism. Change in postoperative refraction from 1 month to 5 years postoperatively was -0.22 ±0.30 D. We found that neither significant endothelial cell loss nor significant intraocular pressure rise occurred in any case throughout the 5-year observation period. No subcapsular cataract presence has been found in a study group of eyes.
Conclusions
Phakic Evo – ICL implantation offered good long-term outcomes for all measures of safety, predictability, and stability for correcting moderate to high myopia and astigmatism.
