Measuring Changes In Iridocorneal Angles After Evo Ic Phakic Lens Implantation With Ss-Oct Anterion

Purpose

The implantation of EVO ICL phakic lenses for the correction of ametropia is increasingly the technique of choice. Once the lens is implanted in its position in the sulcus, the surrounding ocular structures could undergo modifications in their parameters. One of these structures are the iridocorneal angles. Changes in the opening of these angular parameters could lead to the appearance of unwanted secondary phenomena. The purpose of this study is to understand how the temporal and nasal iridocorneal angular structures vary over time once the EVO ICL lens is implanted.

Setting

Clínica Rementería, Madrid- Spain

Methods

Retrospective observational analytical study. Preoperative data are compared with postoperative data (week and month) of patients undergoing refractive surgery with EVO ICL implantation between 2019 and 2022 for the correction of myopia and/or astigmatism. The sample size calculation was performed using the GRANMO software v.7.12. The SS-OCT ANTERION is used to obtain the angular measurements ACA500N, ACA500T, SSA500N, SSA500T, TISA500N, and TISA500T, all measured at 500µm from the sclerocorneal spur, both preoperatively and one week and one month postoperatively. The Anterior Vault (AV) and Posterior Vault (PV) are measured one week and one month post-surgery. The database was analyzed with SPSS 22.0 software (SPSS Inc, Chicago, IL). 

Results

35 right eyes analyzed.Respectively At the preop,1W and 1M postop,values are ACA500N 58.71±11.64º,36.31±7.13º,36.00±7.74º;ACA500T 54.91±11.50º,32.86±7.48º,32.62±7.72º;SSA500N 59.40±9.09º,38.77±8.62º,37.88±8.43º; SSA500T 59.34±9.34º, 35.97±9.09,35.65±9.18;TISA500N 0.31±0.12mm2,0.15±0.05,0.15±0.05;TISA500T 0.31±0.12mm2,0.15±0.06,0.14±0,05 were obtained.A decrease of  -23.20±8.60ºACA500N -22.68±9.80ºACA500T -21.97±6.24ºSSA500N -24.22±7.64ºSSA500T -0.167±0.078mm2TISA500N and -0.18±0.09mm2TISA500T is found (p<0.001).

1W and 1M postop,the AV value is 25558.3±237.4 µm and 2604.6±214.6µm respectively; and the PV of 466.69±189.04 and 428.91±171.34 respectively.An increase in AV of 46.3±82.6 and a decrease in PV of -37.77±78.98 is found.

Conclusions

All angular structures measured at 500µm from the sclerocorneal spur one week and one month postoperatively tend to decrease. Changes in these same structures between one week and one month are minimal.
