Chemical Burn From Blindness To Hope

Purpose
To discuss the management of a case of bilateral severe chemical burn injury followed by severe limbal stem cells destruction using “ Boston type -1 keratoprothesis”.
Setting
Alwatany Eye hospital, Cairo, Egypt
Report of case
A 22 years old male patient presented to our emergency with bilateral severe chemical burn injury , his left eye was more affected .
The 1st line of management was done ( wash with 3 bottles of saline and lid eversion).
His visual acuity at time of presentation was (0.4) (0.1) respectively , he took initial medical treatment.
With close follow ups he developed bilateral severe ocular surface inflammation
(ciliary injection , persistent epithelial defects more than 50% , multiple pyogenic granulomas ) and his visual acuity dropped to (CF 1 m ) ( HM) respectively .
After continuous six months of medical management in the ocular surface clinic he had healing of the ocular surface with dense conjunctival membranes creeping over the cornea (conjunctivalization) and Synechial adhesions of the fornix with Symblepharon .
He went for bilateral amniotic membrane transplantation to reform the fornices and relieve the adhesions then eight months later he went for Right penetrating keratoplasty and Left Boston type 1 Keratoprosthesis surgeries.
Postoperative follow ups showed his visual acuity was (0.2) (0.7) respectively .
Conclusion/Take home message
Ocular Chemical Burn is a devastating , sight threatening emergency that needs a strict management and fortunately keratoprothesis restored the hope to give a blind patient vision and life but it is a high maintenance procedure that requires a lifelong treatment and Follow ups.
