Purpose

To evaluate the results of ReLEX SMILE under general anesthesia and compare them with the results of correction under standard conditions. To develop recommendations for carrying out correction SMILE “in a dream”. To evaluate long term results in 7 years.

Setting

A retrospective, single-surgeon, consecutive case-series study.

Methods

The study includes 54 eyes for 27 patients undergoing bilateral surgery with SMILE for myopia up to − 9,15D and astigmatism up to −4.25D. A general anesthesia was used for patients who not be able to follow instructions or can’t understand them, there were any problems with the spine or central nervous system, 18 patients were afraid of surgery. In cases of significant angle Kappa and more than 1 D astigmatism, preoperative marking and manual positioning of the eye was performed according an offset and cyclotorsion during docking. General anesthesia was made by fractional injection of «Propofol» with monitoring of blood pressure, heart rate, saturation, and a moistened oxygen supply. 50 patients were included in the control group.

Results

All SMILE in both groups ended with complete lenticular extraction. In the first group, in 2 case the vacuum was released and the RePARE mode was needed. UCVA on the first day was 0.92 in the study group and 1.1 in the control group. There were the same indicators of quality and visual acuity by the end of the first month after surgery in both groups. We also got similar topographic and aberrometric dates for two groups with good centration of the optical zone.

A comparative assessment of keratotopography data of patients after general and local anesthesia did not reveal significant differences.

Conclusions

Recovery after SMILE with general anesthesia takes a little longer time for a number of reasons, but ultimately produces excellent and lasting optical correction results. This option allows ReLEX SMLE be performed without stress for the patient and the surgeon for special cases.
