Iris Cyst In A Post-Trauma Patient From Childhood

Purpose
To report a case of iris inclusion cystin a 42-year-old patient with perforating trauma with a spear in childhood and his treatment.
Setting
Department of Ophthalmology, University Hospital ”Mother Theresa” Tirana, Albania.
Methods
A.K is a 42 years old woman who presented in hospital, with complains such as pain, watering and irritation of the right eye. The patient referred that the child had a perforating trauma with a spear in the right eye. Since that moment, the vision in the right eye is weak according to the patient's reference. From the ophthalmological examination BCVA in OD=Hand motion and in OS=10\10. In the biomicroscope examination, conjunctiva is slightly red, an old nasal iris inclusion cyst was identified, occupying the entire width of the iris.(documentet with photos). Intraocular pressure in OD was 22 mmHg. The patient also performed a B-Scan to examine the posterior segment where the results were within normal parameters.
Results
The patient underwent surgery to remove the iris cyst. The operation consisted of deroofing the cyst and its aspiration, being careful of the risk of hemorrhage from the iris. The patient feels calm and without pain after recovering from surgery. Iris cyst is totally removed (documented with photos). BCVA in the right eye is hand motion due to amblyopia installed since childhood in this eye because of perforative trauma.
Conclusions
There are controversies regarding the best management, and often treatments are advised in a step-wise fashion in terms of invasiveness. Treatment is sometimes indicated to prevent secondary complications. Its surgical removal when the cyst is symptomatic is a safe method if a large amount of viscoelastic is used for better separation of the iris structures from the cyst and thus to minimize the risk of hemorrhage.
