Sequential Implantation Of Two Xen 45 Gel Stents In Primary Open-Angle Glaucoma: A Pilot Study

Purpose

 To describe the safety and efficacy outcomes of double minimally invasive glaucoma surgery implant with a subconjunctival drainage approach: the Xen 45 Gel Stent (Xen).

Setting

Retrospective single-centre case series.

Methods

Retrospective single-centre case series of primary open-angle glaucoma patients with at least six months of follow-up after implantation of a second Xen in the same eye via ab externo technique.
 

Results

Eight pseudophakic eyes of 8 patients were studied. The second Xen was successfully implanted in all eyes without significant intraoperative and postoperative complications. Intraocular pressure (IOP) dropped from 30±4.0 to 22.4±3.8 mmHg after first Xen implant (p= 0.0092). Following the second Xen implant, the IOP dropped to 16.1±3.1 mmHg. IOP did not significantly change between postoperative visits at one month and six months after the second Xen; however, 3 patients needed medical therapy to further reduce the IOP towards the target value.
 

Conclusions

 Sequential implantation of two Xen 45 Gel Stents in the same eye is feasible, showed a favorable safety profile and achieved a further reduction in IOP in primary open-angle glaucoma eyes. Further studies are needed to evaluate the safety and efficacy of the procedure on a larger sample and with long-term follow-up.
