Efficacy Of Ab-Interno Canaloplasty Performed With Or Without Gatt In Cases Of Moderate Versus Severe Glaucoma

Purpose
To compare the outcomes of ab-interno canaloplasty with the iTrack microcatheter (Nova Eye Medical, Fremont, USA) as a standalone procedure or combined with gonioscopy-assisted transluminal trabeculotomy (GATT), in moderate versus severe primary open-angle glaucoma (POAG) eyes.
Setting
A single-center, retrospective case series.
Methods
Patients with uncontrolled (≥18mmHg) POAG and no history of glaucoma surgery, except for SLT or CPC, underwent treatment with either canaloplasty or canaloplasty+GATT. Eyes were categorized into moderate or severe groups and were followed for 24 months
Results
Twenty-one eyes (8 moderate and 13 severe) were included. Mean baseline IOP (mmHg) and number of medications were 21.1±3.1 and 1.4±0.7 (moderate) and 23±6 and 2.5±0.6 (severe). At 24 months mean IOP and mean number of medications reduced in both groups to 12.8±1.2 and 11.8±2.7 respectively, and to 0.6±0.5 and 2±0.8 (p=0.01 among groups).
Conclusions
Canaloplasty resulted in a sustained reduction in IOP and anti-glaucoma medications when performed as a standalone procedure or in combination with GATT in patients with uncontrolled moderate POAG. In eyes with severe POAG, canaloplasty was able to reduce IOP.
