Real World Data Of Outcomes Of Three Different Types Of Micro Incision Glaucoma Surgery (I-Stent/Hydrus/Omni) In A District Hospital

Purpose
This study evaluated the 12 week effectiveness and safety of i-stent inject, Hydrus stent and Omni surgical system with or without phacoemulsification in patients with open-angle glaucoma (OAG)
Setting
Retrospective single centre study done over the period of one year in a district hospital. Analysis included 53 eyes of 50 patients
Methods
Electronic patient records (Medisoft/ Epic) were used to obtain the data of all patients who underwent Microinvasive glaucoma surgery (MIGS) standalone vs combined phacoemulsification . Parameters studied included baseline intraocular pressure (IOP) and number of antiglaucoma eyedrops used preoperatively. Further data was collected at first post op visit, 3 weeks, 6 weeks and 12 weeks. Overall cohort and subgroup analyses were done for both combined and standalone procedure. The significance of the data (p value) was found using the paired t test.
Results
Preoperative Mean baseline IOP in overall cohort was 16.94 +/- 6.86 mm of Hg on 2.29+/-1.04 mean medications (drops) reducing to 13.37 +/-4.26 mm of Hg on 1.510+ 1.74 drops at 12 weeks resulting in 22.78% reduction in IOP from mean baseline IOP. Among the combined procedures, Phaco i-stent showed baseline mean IOP of 16.94+/-2.7 on 2.29+/-1.04 drops reducing to 12.76+/-2.6 on 1.66+/-1.44 drops. Phaco Hydrus showed baseline mean IOP of 17.9+/-3.84 on 1.93+/-1.09 drops reducing to 14.8+/-5.45 on 1.53+/-1.24 drops. Phaco Omni showed baseline mean IOP of 18+/-5.54 on 2.0+/-1.30 drops reducing to 12.08+/-3.19 on 1.41+/-1.03 drops.
Conclusions
Overall cohort showed reduction in IOP by 23.84 % and drops by 23.3%. Omni showed greater efficacy in IOP control , showing a reduction in IOP by 31.41 % and drops by 29.5%. Phaco i-stent showed reduction in IOP by 22.30 % and drops by 28.3% . Phaco Hydrus showed reduction in IOP by 17.76 % and drops by 20.7 %.Standalone Omni procedure showed reduction in IOP by 30.66 % and drops by 38 % whereas standalone Hydrus procedure showed reduction in IOP by 11.78%. 20% reduction in IOP at 12 weeks was observed in 60% of patients with Phaco i-stent, 58% of patients with Phaco Hydrus and 69% of patients with Phaco Omni. In conclusion MIGS combined with cataract surgery was safe and resulted in lowered IOP and medication use post operatively.
