Purpose
Detect the incidence of Descemet membrane detachment post (GATT) Gonioscopy-Assisted Transluminal Trabeculotomy by anterior segment optical coherence tomography in patients with open angle glaucoma.
Setting
the study was conducted at kasr alainy hospital
Methods
This is a prospective clinical study that was conducted on 30 eyes in the interval between June 2022 and January 2023
inclusio criteria: Age 18 – 70 yearswith open angle glaucoma undergoing gonioscopy assisted transluminal trabeculotomy

Preoperative Assessment: of all participants will include:
BCVA, slit lamp examination , gonioscopy
Preoperative anterior segment OCT using SD-OCT: to exclude presence of Descemet membrane detachment for the cases who underwent previous anterior segment surgeries.
- GATT procedure was done
- at first day postoperative anterior segmment OCT was done to visualize the angles to detect if there is DM detachment
- after one week AS-OCT was repeated for cases with DM detachment to check for resolution
Results
the IOP was found to be significantly decresed from 24mmHg to 13 mmHg
success rate was about 75%
Dm detachment was detected in only one case wich resolved completely at the end of the 1st week
Conclusions
Ab interno angle surgery has revolutionized glaucoma surgery with the goal of reducing failure rates and bleb-related complications.
GATT is a an effective conjunctival-sparing surgery in lowering the intraocular pressure and numder of glaucoma medications for various forms of open-angle glaucoma.
GATT is a safe surgical procedure with low complications rate.
Descemet membrane detachment is a rare complication post GATT and can heal spontaneously without surgical intervention
