Case Of Lens Coloboma And Congenital Cataract In 3 Year Old Child

Purpose
To evaluate outcomes of cataract surgery in a case of lens coloboma and congenital cataract in 3 year old child.
Setting
Eye Clinic, University Medical Center Zvezdara, Belgrade
Report of case
After creating main incision and staining the capsule, capsular defect in the projection of coloboma was noted with the prolapsed and incarcerated vitreous. Capsular hook to support the lens was introduced and continuous capsulorhexis was performed. Gentle hydrodissection and hydrodelineation was done and the lens matter with the cannula was aspirated. As you can see, lens matter was removed completely but now we were left with a contracted and small capsular bag, so the decision was whether the IOL will be placed in the bag or not. Posterior capsulotomy was performed and enlarged with 25G cutter and was gently implanted in the capsular bag. Immediately the stability of anterior segment was achieved, so the capsular hook was removed, viscoelastic was irrigated and case finished with hydrodissection of the incisions.
Conclusion/Take home message
In cases of coloboma lens phacoemulsification can be successfully performed and is the method of choice.
