Purpose
To present a case 13 year old male with uncontroled secondary glaucoma to Axenfeld rieger syndrome.
Past ocular history Congenital Glaucoma, Trabeculotomy at 11 months OU, Trabeculectomy at 9 years OU.

Report of case
The patient was referred from public health system . IOP was 40mmhg OD with maximal medical therapy and acetazolamide and advanced glaucoma damage. (OS IOP was controlled 10mmhg). we did 2 sessions of Transcleral diode with IRIDEX Gprobe Illuminate. IOP came down to 28mmhg with the same medication.
We decided to perform a phaco combined with ciliary sulcus Ahmed valve tube placement due to complete synechial angle closure and anterior synechiae with shallow anterior chamber.
Conclusion/Take home message
Secondary Axenfeld Rieger Glauocoma represents a therapeutic challenge due to anatomical distortion of the anterior segment.
Four years later, the introocular pressure continue in 12 mmHg and no progression.
