Purpose

We want to present a case of 62-year-old female patient, who underwent cataract surgery with RayOne EMV Toric (RAO210) in one eye and RayOne EMV (RAO200E) implanted in the other eye.

Setting

District Health Care Facility Complex in Bedzin, Ophthalmology Ward, Poland

Report of case

Toric IOLs play an important role for patients to achieve spectacle independence at distance after cataract surgery. The 62-year-old female cataract patient expressed the strong wish for more spectacle independence at intermediate vision, which would allow her to exhibit most of her daily tasks without spectacles. Therefore, the implantation of the enhanced monofocal IOL, RayOne EMV RAO200E (Rayner) was anticipated with the implantation of the toric version, the new RayOne EMV Toric RAO210T (Rayner) in the eye with significant preoperative corneal astigmatism.  

Preoperatively, 1 day, 2 weeks, 2 months and 6 months after procedure we assessed CDVA, UDVA, UIVA, UNVA. Corneal astigmatism of -4.28 D Cyl. Ax. 5 degrees was presented in the left eye (OS: R1:8.35 mm/40.42 D at 5 degrees, R2: 7.55 mm/44.70 D at 95 degrees, R/SE 7.95 mm/42.56 D) and after IOL calculation the IOL power with +15.75 D Sphere, 4.5 D Cyl., SE: 18.0 D for RayOne EMV Toric was chosen and the toric marks were aligned at 95 degrees. The post-op expected residual refractive outcome was +0.38 D sphere and -0.11 D Cyl. Right eye (OD) underwent the cataract procedure first with +20.5 D RayOne EMV implantation. Baseline CDVA was 0.30 logMAR (OD) and 1.0 logMAR (OS) and 2 months after procedure increased to 0.10 logMAR (OD) and 0.18 logMAR (OS). Monocular UIVA and UNVA 2 months post-op was 0.60 and 0.70 logMAR (OD),0.50 logMAR and 0.54 logMAR (OS). Time interval between procedures was 6 weeks.

Conclusion/Take home message

RayOne EMV Toric IOL implantation seems to be a promising alternative to the traditional procedure of standard monofocal toric IOL implantation in patients with preoperative corneal astigmatism to improve their . Further study is required to evaluate long term results in a bigger cohort
