Use Of An Edof Iol (Vivity Iol) In Patients After Retinal Detachment Repair

Purpose

To report the performance of the AcrySof IQ Vivity EDOF intraocular lens in patients with cataract after pars plana vitrectomy (ppV) due to rhegmatogenous retinal detachment (RRD).

Setting

Prospective, non-randomized, monocentric clinical study at the Department of Ophthalmology, University Eye Hospital Ulm, Ulm, Germany. 

Methods

Patients with cataract who underwent successful primary ppV (within 3 days if macula was involved) due to RRD at least 3 months ago were included in the study. Exclusion criteria were: High ammetropia, corneal astigmatism >3D, other corneal pathology, epiretinal membrane, glaucoma, uveitis, other retinal pathology. Both eyes were enrolled unless patient denied surgery in the non-RRD affected eye. Six months postoperatively corrected and uncorrected distance (4m), intermediate (66cm) and near (40cm) visual acuity (CDVA, CIVA, CNVA, UDVA, UIVA, UNVA), macular OCT and pupillometry will be performed. Moreover, patient satisfaction and halos are evaluated with a standardized questionnaire (IOLSAT and Catquest) and a “simulation” test.

Results

Up to now 28 eyes (15 vitrectomized; 8 macula-off) of 15 patients (59,6±6,1 y.o.) are enrolled. 13 Patients underwent bilateral surgery. Mean initial CDVA before vitrectomy was 1.06±0.88 LogMAR, before cataract surgery 0.39±0.22, improved to 0.21±0.18 one day and to 0.13±0.17 one month after surgery. Mean UDVA was 1.13±0.66 LogMAR before cataract surgery, 0.32±0.24 one day and 0.22±0.25 one month after surgery. CDVA of non-vitrectomized eye remained stable (from 0±0.12LogMAR to 0±14) one month after surgery, whereas UDVA improved from 0.5±0.36 LogMAR to 0.12±0.2, respectively. Mean refraction changed in the vitrectomized eye from -4.32±3.6dpt to -0.13±0.62dpt one month after surgery and from -1.93±2.85 to -0.05±0.19dpt in the fellow eye.

Conclusions

The EDOF Vivity intraocular lens shows sufficient uncorrected and corrected postoperative distance VA in vitrectomized eyes after pars plana vitrectomy for retinal detachment, in eyes that are successfully treated within three days of macular involvement. Further results of the study are needed to show the performance of the IOL in the intermediate and near distance, as well as the patient satisfaction.
