Comparative Analysis Of Ocular Biometry Measurements: Iol Master 700 And Anterion

Purpose

This study aims to compare ocular biometric measurements between the IOL Master 700 and ANTERION devices.

Setting

Department of Ophthalmology, Unidade Local de Saúde de Gaia e Espinho.

Methods

Ninety-eight eyes from 49 cataract surgery patients were evaluated between June 2023 and January 2024. Parameters such as anterior keratometry (Steep K, Flat K) with axis, Simulated Keratometry (Sim K), Central Corneal Thickness (CCT), Aqueous Depth (AQD), Lens Thickness (LT), Axial Length (AL), and White-to-White distance (WTW) were measured using both IOL Master 700 and ANTERION. Agreement between devices was assessed using Bland-Altman analysis with 95% limits of agreement (LoA) and Intraclass Correlation Coefficient (ICC).

Results

Mean differences for Sim K, Steep K, and Flat K between ANTERION and IOL Master 700 were -0.13 ± 0.18 D, -0.17 ± 0.47 D, and -0.11 ± 0.26 D, respectively. Excellent agreement was observed for Sim K (ICC=0.976) and Flat K (ICC=0.964), while Steep K showed very good agreement (ICC=0.896). Steep K Axis displayed moderate to good agreement (ICC=0.675). Both AL (ICC=0.997) and CCT (ICC=0.936) showed excellent agreement. AQD (ICC=0.846) and WTW (ICC=0.802) revealed good agreement, with mean differences of 0.03 ± 0.11 mm and -0.11 ± 0.19 mm, respectively. LT presented a low agreement rate (ICC=0.317) with a mean difference of +0.34 ± 0.44 mm, indicating higher values measured by ANTERION.

Conclusions

ANTERION generally aligns well with the IOL Master 700 in measuring ocular biometry, albeit with a slight tendency to present lower anterior keratometry values and WTW measurements. The consistency in Lens Thickness estimates was poor, showing significantly higher values with ANTERION. Further extensive studies are required to reinforce the evidence on the consistency between these two devices, particularly concerning LT.
