Comparison Of Visual Function Between Two Presbyopia Correcting Intraocular Lenses: A Randomized Bilateral Study

Purpose

To evaluate visual acuity and defocus curves in patients bilaterally implanted with trifocal (AcrySof IQ PanOptix) intraocular lens (IOL) or extended depth of focus/bifocal (TECNIS Synergy) IOL.

Setting

A prospective, multicenter, randomized, double-masked, bilateral study. 

Methods

A prospective, multicenter, randomized, double-masked, bilateral study was conducted in patients (aged ≥22 years) presented for routine cataract surgery. Patients with a projected uncorrected distance visual acuity (VA) better than 0.10 logMAR postoperatively were randomized (1:1) to PanOptix (P) or Synergy (S) IOL bilateral implantation. Study-wide data were evaluated postoperatively for binocular uncorrected and distance-corrected VA (logMAR) at distance (4 M; UCDVA, BCDVA), intermediate (60 cm; UCIVA, DCIVA), and near (40 cm and 33 cm; UCNVA, DCNVA) in both photopic and mesopic conditions at 1, 3 and 6 months; and defocus curves at 3 and 6 months.

Results

At 6 months (P, n=138; S, n=138), photopic binocular UCDVA 4 M, UCIVA 60 cm, UCNVA 40 cm, and UCNVA 33 cm (logMAR): (P) 0.02±0.11, 0.09±0.14, 0.15±0.15, and 0.22±0.13; (S) 0.05±0.09, 0.09±0.12, 0.14±0.11, and 0.23±0.11. Photopic BCDVA 4 M, DCIVA 60 cm, DCNVA 40 cm, and DCNVA 33 cm: (P) -0.02±0.08, 0.12±0.14, 0.13±0.12, and 0.21±0.13; (S) -0.01±0.09, 0.12±0.14, 0.13±0.12, and 0.19±0.12. Mesopic intermediate and near VA data were also similar between P and S. P presented non-inferior VAs to S at all measured distances. The defocus curves showed that P and S had VA of ≤0.16 logMAR from -3.00 D to +0.50 D, with no statistically significant difference between the two groups (P>0.05).

Conclusions

The study results demonstrated that PanOptix IOL provided a comparable range of vision to Synergy IOL from distance to near (33 cm) under photopic condition. Under mesopic condition, both lenses provided similar visual acuities at intermediate and near distances.
