Purpose

To compare outcomes with TECNIS Eyhance Optiblue (Eyhance) or TECNIS 1 Piece lenses (TECNIS 1) in cases with combined cataract and vitrectomy surgery for epiretinal membrane (ERM).

Setting

Multicenter prospective case-control study

Methods

Patients at Nagoya University Hospital, Miyake Hospital, Ichinomiya Municipal Hospital, Kariya Toyota General Hospital, and Nishio Municipal Hospital were included. Patients underwent combined cataract and vitrectomy surgery and used either Eyhance or TECNIS 1. The primary endpoints were distance-corrected distance visual acuity and distance-corrected intermediate-distance visual acuity at 3 months postoperatively, and secondary endpoints were macular manipulation surgery time for ERM/internal limiting membrane peeling, and patient satisfaction as assessed by Rasch analysis from Japanese version of Catquest-9SF results.

Results

Overall, 141 patients were analyzed, 62 with TECNIS 1 and 79 with Eyhance. Mean postoperative distance visual acuity (logMAR) and intermediate distance visual acuity was 0.11 and 0.15 for TECNIS 1 and 0.09 (P = 0.174) and 0.35 (P = 0.200) for Eyhance, respectively. The mean macular operation time was 6.38 minutes for the TECNIS 1 and 6.16 minutes for the Eyhance (P = 0.680). There was no significant difference in the mean logit of patient ability between the two groups (P = 0.231) in Rasch analysis. When the difference in difficulty of each item of the questionnaire was compared between the two groups as Differential Item Function, the superiority of Eyhance was confirmed in Q2 (0.72, P = 0.0269), Q3-4 (1.2, P = 0.0169).

Conclusions

There were no significant differences in distance-corrected distsnce visual acuity, distance-corrected intermediate-distance visual acuity, or macular manipulation time during surgery between the two groups. Patient satisfaction based on Japanese version of Catquest-9SF questionnaires was higher in Eyhance.
