Purpose

To compare the visual outcomes and the optical quality in patients with bilateral implantation of Tecnis® Synergy or AcrySof® PanOptix trifocal IOL.

Setting

We retrospectively analyzed 64 eyes of 32 patients who underwent bilateral cataract surgery with Tecnis® Synergy or AcrySof® PanOptix IOL at Samsung Medical Center between February 2021 and March 2024.

Methods

Preoperative and postoperative manifest refraction, refractive error, monocular uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA) were recorded. Binocular defocus curve, contrast sensitivity, spectacle independence, satisfaction, and visual symptoms were analyzed at 3 months postoperatively.

Results

Each 16 patients were included in Synergy group and Panoptix group, the mean monocular postoperative visual acuities at all distances and refractive outcomes and refractive errors (MAE) were comparable between each group (p>0.05). In defocus curve, Compared to Panoptix group, Synergy group provided better visual acuity from -3 to -4D defocus steps. Panoptix group showed better visual acuity from plano to -2.5D, not statistically significant (p>0.05). Contrast sensitivity before and after surgery did not deviate significantly from the standard in both groups. Glare symptom score was higher in Panoptix group than Synergy group (p<0.05), there was no significant difference in Halo, Spectacle independence and satisfaction at near distance.

Conclusions

Comparing the visual outcomes and the optical quality in patients with bilateral implantation of Synergy IOL and PanOptix IOL, all two groups showed good visual acuity at intermediate distance and near distance. Panoptix group showed better visual acuity up to -3D defocus step and Synergy group showed better visual acuity within -3D defocus step. Visual quality was comparable in both groups. Continuous follow-up is needed to confirm long-term clinical results in the future, and studies targeting more patients are needed.
